Must contact each DOT-regulated employer from past 3 years within 30 days of hire. Must document responses.
Current CDL copy on file. Endorsements verified against vehicle types operated. License number tracked.
Required for drivers who obtained CDL after February 7, 2022. Training Provider Registry completion certificate.
Annual certification of violations (or no violations) for the preceding 12 months. Driver-signed, dated, filed.
Security threat assessment records, training documentation for applicable drivers transporting HAZMAT.
